What caused my internal fork damage?

Just want to get some suggestions from yall on what to look for on what could have caused the issue with the metal washer/spacer under the fork seal. I'll post a picture later but it can be seen under my build page. The washer is no longer flat but concaved. After putting the fork back together, the one that had the issue is not as smooth at the very bottom of the stroke when compared to the other side...it almost sticks at full compression (no oil in these yet cause I have a feeling I might have to take them apart again to figure this out). I did do a pretty good inspection of them while apart but saw nothing that raised any concerns. The only think I can think of is that the dampening rod spacer (between the lower leg and rod may be smashed and that could have caused a hard impact during full compression...but I don't know). Throw me some suggestions on what could have caused it and what to look for to make sure it won't leak and cause the same issue.
